Most adult family members can be hired and paid to provide the care. A spouse can also be the paid caregiver.

Why the spouse question is different
Federal Medicaid rule treats a spouse (and a parent paying for a minor child) as a “legally responsible relative”, someone already assumed to owe the care for free, so paying them is barred by default. A state has to build an explicit exception into its waiver or state-plan option to pay a spouse at all; the rule for any other relative (an adult child, a sibling, a grandchild) is looser and usually allowed. That is why the two rows above can carry different answers on the same page.
The going rate
$77.54–$133.44/day (Level 1-3), paid to the contracted provider agency, with at least 60% required to pass through to the caregiver. “$77.54/day Level 1, $99.71/day Level 2, $133.44/day Level 3 … The minimum percentage passthrough for Structured Family Caregiving services is 60%.”
How it works
The Medicaid recipient must be on the Health & Wellness, TBI, or PathWays for Aging waiver and live with the caregiver; care goes through a contracted SFC provider agency, which enrolls the family caregiver and pays a daily stipend (at least 60% of the state’s per-day rate) rather than an hourly wage. A spouse can be paid this way even though a spouse may NOT be paid under the separate hourly Attendant Care service.
What this doesn’t answer
The exact stipend an individual caregiver actually receives (versus the rate FSSA pays the provider agency) was not separately published in the materials found. Current SFC waitlist status was not found this session.
