How Iowa pays a family caregiver

Why the spouse question is different
Federal Medicaid rule treats a spouse (and a parent paying for a minor child) as a “legally responsible relative”, someone already assumed to owe the care for free, so paying them is barred by default. A state has to build an explicit exception into its waiver or state-plan option to pay a spouse at all; the rule for any other relative (an adult child, a sibling, a grandchild) is looser and usually allowed. That is why the two rows above can carry different answers on the same page.

How it works
The Medicaid member (or their representative) adds CDAC to their HCBS waiver service plan and acts as the employer who finds, hires, and trains the attendant, who can be an adult family member other than a spouse or a minor’s parent/stepparent; a case manager or DHS service worker must approve the CDAC agreement, and a financial/fiscal agent processes the paycheck.
What this doesn’t answer
The $6.02-per-15-minute figure is the state’s maximum reimbursement (upper rate limit), not necessarily the exact negotiated wage a specific family caregiver receives. Current CDAC waitlist status was not found in the materials reviewed.
