Why the spouse question is different
Federal Medicaid rule treats a spouse (and a parent paying for a minor child) as a “legally responsible relative”, someone already assumed to owe the care for free, so paying them is barred by default. A state has to build an explicit exception into its waiver or state-plan option to pay a spouse at all; the rule for any other relative (an adult child, a sibling, a grandchild) is looser and usually allowed. That is why the two rows above can carry different answers on the same page.

How it works
Applicants are referred into the IL Waiver through MDRS, which maintains a statewide referral database ordered by date of referral; the participant selects their own personal care attendant, and a chosen relative (if not a legally responsible individual and not living in the home) must meet all Medicaid provider standards and be certified competent before being paid.
What this doesn’t answer
Whether a comparable family-hire provision extends to Mississippi’s other HCBS waivers (Elderly and Disabled, Assisted Living, TBI/SCI, ID/DD) was not confirmed. Their public program pages do not address family/spouse hiring at all, so this page is scoped to the IL Waiver only. The $4.14/15-min figure is the billed Medicaid reimbursement rate, not a confirmed net hourly wage after any fiscal-agent fee. No explicit waitlist length was found.
