Fee: $25 non-refundable filing fee. Alaska requires no tax-clearance certificate to accept the filing. Until that filing is accepted the LLC still legally exists — and whatever Alaska charges an LLC each year keeps accruing against it.

The form itself (read in full) requires attestations that the entity is in good standing and all biennial reports due have been filed and paid; it ‘will not be filed’ if the signer does not match an official on record or if the biennial report is not current. Tax clearance in Alaska
No tax-clearance certificate is required to file in Alaska. That is not the same as owing nothing — it means the state will accept the filing without a revenue-agency sign-off first.
Form 08-490 requires only good standing and current biennial reports (AS 10.50.430 attestations) — no Department of Revenue clearance is required or mentioned. Alaska has no state individual income tax or statewide sales tax, so most pass-through LLCs have no DOR account to clear.
Do not just walk away
Closing the business is not closing the entity. Failure to file the biennial report leads to involuntary dissolution under AS 10.50.408: the Division mails a notice of delinquency; per secondary corroboration, if the LLC does not respond within 60 days of mailing and does not file within about six months of the due date it is involuntarily dissolved. Reinstatement is available within two years by filing all missed reports and paying all fees/penalties; the entity name is protected only ~6 months after dissolution.

Closing the tax accounts
Cancel the Alaska business license by filing Business License: Request to Cancel, Form 08-4732 (per the reminder printed on Form 08-490 itself; see BusinessLicense.Alaska.Gov). If the LLC is taxed as a corporation, file a final Alaska corporate net income tax return with the Alaska DOR; pass-through LLCs generally have no state income tax filing.
