Why the spouse question is different
Federal Medicaid rule treats a spouse (and a parent paying for a minor child) as a “legally responsible relative”, someone already assumed to owe the care for free, so paying them is barred by default. A state has to build an explicit exception into its waiver or state-plan option to pay a spouse at all; the rule for any other relative (an adult child, a sibling, a grandchild) is looser and usually allowed. That is why the two rows above can carry different answers on the same page.

How it works
The MassHealth member must be found PCA-eligible and works with a state-contracted Personal Care Management (PCM) agency, which assesses need and submits the prior-authorization request to MassHealth; the member is the legal employer, recruits/hires their own PCA (a relative is fine unless excluded), and payroll runs through a state fiscal intermediary. New PCAs, including relatives, must complete a one-time New Hire Orientation.
What this doesn’t answer
No current official page states a plain current-dollar base hourly wage; the only live pages with an explicit dollar figure are dated 2019-2021 collective-bargaining figures, and a November 2025 rate notice gives only blended per-15-minute Medicaid billing units rather than a clean take-home wage, so the rate is carried as unverified rather than the stale 2021 figure. Whether there is any enrollment cap or waitlist was not confirmed either way.
