Updated August 7, 2026. Quick answer: Medicare generally stops at the border. Some Medigap plans add a foreign-travel emergency benefit — and 🔴 it pays 80% after a $250 deductible, only for care beginning in the first 60 days of a trip, up to a lifetime maximum of $50,000. Not per trip. Lifetime.

The general rule first

Original Medicare generally does not cover health care outside the United States, with a small number of narrow exceptions. For a traveller the practical translation is: assume nothing is covered, and treat any exception as a surprise rather than a plan.

What the Medigap benefit actually is

ElementThe rule
Share paid80% of billed charges for covered care
Deductible$250
WindowCare beginning in the first 60 days of the trip
🔴 Maximum$50,000 LIFETIME

🔴 The lifetime cap is the number people misread, and it is the one that matters. It is not $50,000 per trip or per year. It is $50,000 for the rest of your life — and one air ambulance repatriation can approach or exceed it on its own.

⚠️ The 60-day window is the second trap, and it bites the people most likely to rely on the benefit: extended winter stays and long trips. Care beginning on day 61 is outside it.

So is it enough?

For a two-week trip with an ordinary emergency, frequently yes. For a serious event requiring evacuation, or for a trip longer than two months, the honest answer is that the Medigap benefit is a partial backstop rather than cover. That is the gap standalone travel medical policies are sold into, and it is a real gap rather than a manufactured one.

The questions that decide it: how long is the trip, does the policy cover evacuation separately from treatment, and have you used any of the $50,000 before?

⚠️ And this is about travelling, not moving. If the plan is to live abroad, the rules are different — Social Security and Medicare when you live abroad.
