What happens if you miss it

$50 late filing penalty, in addition to the annual report filing fee. (Title 31 §1667 / §1680)

What eventually happens if it stays unfiled: Non-payment of the annual report fee or the $50 penalty leads directly to administrative dissolution (domestic) / revocation (foreign); no fixed grace period stated beyond ‘prior to revocation or administrative dissolution.’ (Title 31 §1667) “Upon a limited liability company’s failure to file the annual report and to pay the annual report fee or the penalty, the Secretary of State shall… administratively dissolve a limited liability company.”
