Form 1095-A VOID, CORRECTED or Missing: What to Do

Updated October 17, 2026. Quick answer: A VOID box means the form was sent in error and neither it nor the earlier one should feed Form 8962. A CORRECTED box means this form replaces the original. If you expected a form and have not received one by early February, the Form 8962 instructions say to contact the Marketplace.

Form 8962 (premium tax credit) for 2025, read line by line: Form 8962, Form 8962 lines 9 and 10 and Form 8962 lines 11 to 23.

The two boxes at the top of the form

Form 1095-A has two check boxes above Part I, VOID and CORRECTED. The recipient instructions explain each one. They matter because Form 8962 reads the figures from the form you should be using. The overview of the set is Form 1095 explained.

The VOID box

The recipient instructions say “If the “VOID” box is checked at the top of the form, you previously received a Form 1095-A for the policy described in Part I.” They continue: “That Form 1095-A was sent in error.” and “You shouldn’t have received a Form 1095-A for this policy.” The instruction is “Don’t use the information on this or the previously received Form 1095-A to figure your premium tax credit on Form 8962.”

The Form 8962 instructions say the same: “Do not use the information on the Form 1095-A with the “VOID” box checked or the previously received Form 1095-A to complete Form 8962.”

Why a VOID statement is sent

The filer instructions give the examples: “If a Form 1095-A was sent for a policy that shouldn’t be reported on a Form 1095-A, such as a stand-alone dental plan or a catastrophic health plan, send a duplicate of that Form 1095-A and check the VOID box at the top of the form.” On your side, the recipient instructions say “If you or your family members were enrolled in a Marketplace catastrophic health plan or separate dental policy, you aren’t entitled to take a premium tax credit for this coverage when you file your return, even if you received a Form 1095-A for this coverage.” The dental amounts that do belong on a policy form are explained on the Form 1095-A column C page.

The CORRECTED box

The recipient instructions say “If the “CORRECTED” box is checked at the top of the form, use the information on this Form 1095-A to figure the premium tax credit and reconcile any advance credit payments on Form 8962.” The next sentence is the contrast: “Don’t use the information on the original Form 1095-A you received for this policy.”

The filer’s side is “Report corrected information on the Form 1095-A to the IRS and to the recipient as soon as possible after discovering that information reported is incorrect.” “Check the CORRECTED box on the top of the form.”

If you think the information is wrong

Before any box is involved, the form tells you where to go: “Check the information on this form carefully.” “If you think the information is incorrect, or if you think you should not have received a Form 1095-A because neither you nor anyone else in your family was enrolled in Marketplace health insurance, please contact your Marketplace Call Center.”

A blank or wrong column B is a separate case. The Form 8962 instructions say “You do not have to request a corrected Form 1095-A from the Marketplace.” for the benchmark premium. That case is on the Form 1095-A column B page.

If you did not receive a Form 1095-A

The Form 8962 instructions give a date and a step: “For coverage in 2025, the Marketplace is required to provide or send Form 1095-A to the individual(s) identified in the Marketplace enrollment application by January 31, 2026.” Then “If you are expecting to receive Form 1095-A for a qualified health plan and you do not receive it by early February, contact the Marketplace.”

The IRS site does not supply a form for you to fill in. The cover note on the IRS copy says “As the form is to be completed by the Marketplaces, individuals cannot complete and use Form 1095-A available on IRS.gov.” The same cover note explains who completes it: “Individuals receiving a completed Form 1095-A from the Health Insurance Marketplace will use the information received on the form”.

If the form went to someone else

The Form 8962 instructions cover shared policies: “Under certain circumstances, for example, where two spouses enroll in a qualified health plan and divorce during the year, the Marketplace will provide Form 1095-A to one taxpayer, but another taxpayer will also need the information from that form to complete Form 8962.” The remedy follows: “The recipient of Form 1095-A should provide a copy to other taxpayers as needed.”

For a covered person outside your tax family, the Form 1095-A Parts I and II page explains why they receive a separate form. Once you have the right form, the Form 1095-A line 33 page shows how its totals reach Form 8962, which is filed with Form 1040 or Form 1040-SR.

More than one policy

A family can hold several forms at once. The recipient instructions say “you will receive a Form 1095-A for each policy.” Each form refers to its own policy, and the Form 1095-A Parts I and II page covers the line 2 policy number that tells them apart. If you are reading this because forms disagree, check the policy number in Part I before deciding which one is VOID or CORRECTED. For the premium credit concepts, the ACA subsidy calculator page is separate reading.

Edition and tax year

The 2025 Form 1095-A and the 2025 Instructions for Form 1095-A were the ones read for this page, along with the 2025 Form 8962 and its instructions. They were the current files on irs.gov on 2026-10-17. Check the tax year printed on the form you received.

Common mistakes

  • Using a VOID form anyway. The instructions say “Don’t use the information on this or the previously received Form 1095-A to figure your premium tax credit on Form 8962.”
  • Using the original form after a corrected one arrives. The instructions say “Don’t use the information on the original Form 1095-A you received for this policy.”
  • Waiting without contacting anyone when no form comes. The Form 8962 instructions say “you do not receive it by early February, contact the Marketplace.”
